There are adjustments that can be made to cleats to prevent a rider from developing discomfort on the outside of the foot while cycling. For example, outward tuned position of the cleats should be avoided and vice versa, excessive im-toe in-cleat orientation with collamation downward twist should be supplanted by more pronation movement.

In order to relieve discomfort on the lateral aspect of the foot, it is recommended to place the cleats the most rearward part of the shoe so as to lower the ankle joint and relax the forefoot area. as cleat is shifted towards the middle of the shoe volume it increases the stance on the pedals and enhances hip rotation at the same time distributing less weight on the lateral foot. This alteration assists in maintaining optimal muscle activations and foot positioning on the pedals to enhance power generation.

Notice pain in the outside of my foot especially with sweetspot or higher effort type rides. Typically only the right foot which I believe has an anatomic long leg
